An assessment of the client's total needs will be completed and then develop a care plan to address the areas of concern.  We will monitor the client's well being and conduct home visits to observe safety and provide support.

Ensuring that the client's and family needs are meet and their best interest is a priority as well as supporting your health care wishes. Attending doctor appointments or care plan meetings at the rehab center or assisted living to ensure services are provided and needs addressed.

Crisis Management

Crisis intervention helps clients navigate through emergency departments and hospital stays, rehabilitation stays, and ensuring that adequate care is available to the client. For families that live out of town this can be a much-needed 24/7 emergency contact. 

Coordination of Services

Ensure services are in place and followed through as well as ensuring  services or procedures are not duplicated.  Continuity of care  communications are coordinated between family members, doctors and other professionals and service providers.

Placement Assistance

When the time comes when it is no longer an option to remain in the home safely, we will assist with the transition to a retirement community, Assisted Living or Skilled Nursing Facility.  Finding a community that is the right fit for you.  

Education and referrals for local resources

Education provided to families regarding Alzheimer's disease, the different levels of care available, coping with your parents aging process, etc.  We can provide resources in the community such as: attorney, financial planner, handyman, housekeeper, organizer, landscaping, home health, companion care, etc.
